NOTE: No class on  April 26, April 27, May 24, May 25 Year End Recital 7:00pm, June 13, Place TBA	Spring Semester
CLASSES	FEE:
Beginner (4:30 – 5:30pm, Sun)	$170.00
Intermediate (9:30 – 10:45am, Sat)	$212.50
Advanced (11:00am – 12:15pm, Sat)	$212.50
Apprentice (5:45 – 7:15pm, Sun)	$255.00
Junior A (3:30 – 5:00pm, Sat)	$255.00
Junior B (5:15 – 6:45pm, Sat)	$255.00
Senior (12:30 – 2:30pm, Sat)	$255.00
REGISTRATION FEE (apply only for new students)	$15.00
LATE FEE (after February 2, 2008)	$10.00

-The student has to be 5 & up.
-Make checks payable to Angel Performance Art.
-Placement test will be given to new students at 9:00am on Feb 2. We reserve the right to refuse the admission.
-No refunds after the class starts except for medical reasons documented by a doctor's note. No exceptions.
-Class family discount (siblings): 1st child pays 100%, 2nd child - 5% discount, 3rd child - 10% discount.

RELEASE FORM: I hereby absolve Angel Performance Art staff and all related agents of all responsibilities for injury or illness to the above student and hold the above parties and their agents harmless from all claims. All students participate at their own risk.

PHOTO RELEASE: I hereby agree that any photographs or video tapes of the above student, taken by Angel Performance Art or any authorized party, may be used exclusively for any purpose by Angel Performance Art or any other use authorized by Angel Performance Art.
